Form 1099-HC is an important form that’s unique to Massachusetts. It reports proof of health insurance coverage that meets Massachusetts Minimum Creditable Coverage (MCC) standards for residents and certain part-year residents each year. It also indicates whether the coverage met MCC requirements.
According to the Massachusetts Health Reform Law which was passed on January 1, 2006, all Massachusetts residents who are above the age of 18 must have health insurance if it is affordable to them, and the coverage must meet MCC standards to avoid Massachusetts individual mandate penalties. The residents use this form to complete Schedule HC when filing their Massachusetts state income tax return.
Basically, it is a year-end form that ensures the right coverage information is properly reported to both the individual and the Massachusetts Department of Revenue (DOR).
The filing is generally done by the Massachusetts health insurance carriers on behalf of employers. But in some cases, employers or third-party reporting agents also issue the form to the covered individuals and submit the form to the Massachusetts DOR.
For employers with fully insured plans, the insurance carrier generally takes on the responsibility of submitting Form MA 1099-HC to the Massachusetts DOR and distributing copies to covered employees. However, if the carrier fails to do so, the employer should ensure the reporting requirements are met.

Here’s all the information you need in order to file Form MA 1099-HC :
Issuer details: The insurance company’s or administrator’s name and Federal Index Number (FID)
Subscriber details: Full name, date of birth (DOB), mailing address, and subscriber number
Coverage details: The months during which the individual was covered by the MCC in a given tax year. Indicate full-year coverage if the employee was covered for all 12 months. If not, check only the boxes for the specific months during which MCC was provided.
Dependent details: Names of any dependents on the policy and their applicable coverage months, if any.
You must meet Massachusetts 1099-HC filing deadlines if you want to stay compliant and avoid penalties. These are the important due dates and the potential consequences for missing them.
If you fail to file or report Form 1099-HC on time, it may result in a penalty of $50 per individual, up to a maximum of $50,000. This includes fines for each form that is not submitted by the applicable deadline. 1. Using the wrong form
A common misconception is that Form 1095-B can be used as a substitute for Form 1099-HC. It cannot. Form 1095-B is a federal health coverage form and does not satisfy Massachusetts state reporting requirements. Always use Form MA 1099-HC for Massachusetts health coverage reporting.
2. Unclear filing responsibility
When it is not clear whether the employer or the carrier is responsible for filing and submitting the form to the DOR, it can cause missed or duplicate reporting. To avoid this, confirm in writing who is responsible for issuing the form and who will handle the DOR submission before the filing process begins.
3. Incorrect coverage months
Reporting the wrong MCC coverage periods is pretty common and consequential, as it directly affects the accuracy of state reporting. Always review full-year and month-by-month MCC status carefully before you file.
4. Missing dependent information
Incomplete dependent data is a frequent trigger for corrected filings. If dependents are covered under the policy, make sure dependent names and applicable coverage months are included accurately before generating the form to avoid this issue.
5. Outdated subscriber records
Including stale names, dates of birth, numbers, or addresses can lead to errors that are preventable. Reconcile dependent coverage records before filing.
6. Late XML validation
When you test the XML output too close to the deadline, it can leave little time to resolve any technical issues. Do the XML testing before the filing window opens so that there’s enough time to identify and correct anything.

Suppose a fully insured employer provides health coverage through a commercial insurance carrier, the carrier must take on the responsibility for issuing Form 1099-HC to the recipients and submitting it to the Massachusetts DOR. However, the employer should still confirm that the carrier is fulfilling these obligations on their behalf.
For self-insured employers who have their own health plans, it’s the employer who has to furnish Form 1099-HC. Here, the employer is responsible for not just sending copies to covered individuals, but also for reporting it directly to the Massachusetts DOR.
