To complete Form 1095-C, employers must provide detailed information about employee health insurance coverage for the year, including:

Employers with 50 or more full-time employees must file Form 1095-C under the ACA to report employee health coverage.
An ALE must file a Form 1095-C for each employee who was a full-time employee of the ALE Member for any month of the calendar year. Generally, the ALE Member is required to furnish a copy of the Form 1095-C (or a substitute form) to the employee.
Filing Form 1095-C helps the IRS determine employees’ eligibility for the premium tax credit if they purchased insurance through the Health Insurance Marketplace and ensures employers avoid potential penalties for non-compliance.
